An Interoperable Social Care Network
The ability to exchange information with other systems, known as interoperability, is important for overall coordination for people in need. An interoperable system means that a doctor using one system of record is able to pull a patient’s medical history from a healthcare provider that uses a different system.
Community-Based Organizations (CBOs) often use different systems as well — and those systems are not always set up to work together. We believe that CBOs should be able to participate, refer, and connect to networked social care from their own case management system.
Integration Choice
In the same way that (CBOs) select their case management systems to suit their unique requirements, they should be able to decide if they want to integrate with other systems.
They should be able to tell other systems whether or not they want to receive referrals, and if so, where referrals should be sent.

Through our API and webhooks, stakeholders can see referral outcomes in real-time.

Our customers have access to assessment history so that they don’t have to repeat the same questions.
